I have read the foregoing, and acknowledge that MetroLiens is under no obligation to file on my behalf until such time as my payment has cleared. By submitting this form, and engaging MetroLiens' services, you hereby appoint MetroLiens, and its agents, as your attorney in fact in connection with the execution and filing of all Mechanic’s Lien documents incorporating the information contained herein and thereby authorize the execution and filing of the same. You further attest to the accuracy of all factual matters set forth herein. Any dispute(s) related to MetroLiens services shall be asserted in writing within thirty (30) days after you become aware of any such dispute, and must be resolved first through mediation, then through non-binding arbitration, prior to the initiation of litigation. You hereby submit to the jurisdiction of the Supreme Court of the State of New York, County of New York in connection with any such litigation.
